Fordland Clinic is committed to the concept of becoming a patient centered medical home, and to that end, has achieved Level 3 recognition through the National Committee For Quality Assurance. Fordland Clinic’s second location, Tri-Lakes Community Health Center, located in Kimberling City, Missouri, is working toward designation through NCQA as well.

In every day terms, participation means that your care will be simpler to manage and include more of a connection with the doctor you choose to serve as your “home” provider. This one provider will have a comprehensive snapshot of your health at all times, including medications, test results, immunizations and other records. He or she will work more closely with you to halt potential health challenges before they grow and support you if you need care from a specialist of any kind.

The provider will serve as one central point of contact for your care team and manage everything from test results, medications and care with specialists of any type. This means a patient would work to develop a trusting relationship with one provider that they choose to coordinate every aspect of their care, over time.

Here at Fordland Clinic the Registered Nurse Care Managers work with your primary provider, as well as other health care providers, to take care of you. Your care team will involve you in decisions about your health and health care, and thus be able to develop a stronger relationship with you. Your Nurse Care Manager will be here to help you every step of the way! They will help you set goals and work toward being healthier. They will also help you with where you need to go in the healthcare system to get the right type of care. It can seem like a big puzzle sometimes, but your Nurse Care Manager will help you figure it out. Getting the right type of care means you will get your care in the right place and at the right time.

Following are a few things your care manager will help with:

  • Identify and achieve your healthcare and wellness goals
  • Assist in helping you manage your health conditions
  • Help you access a family doctor and other medical care providers
  • Schedule appointments, lab work and testing
  • Referral to specialists
  • Education classes or material
  • Coordinate community resources
  • Set health goals and direct you toward the steps needed to reach them
